Known limitation #4 L04
The automatic P leaves out plaque kept aside in N segments: it does not count in the SIS. Check the P manually.

This page lists the known limitations of the cctamap.com tools: what they do not get right yet (open) and what has already been fixed (resolved), with the version and date of the fix.
The open ones warn next to the result when a case activates them: the map shows «⚠ Known limitation #X» under the category and adds the same line to the draft. The warning does not change the category or the report.
The tool is an educational draft: it does not analyze images, it is not a medical device and it does not replace clinical judgment. Verify everything against the original study.
Each one warns next to the result when the case activates it.
The automatic P leaves out plaque kept aside in N segments: it does not count in the SIS. Check the P manually.
With a «Direct:» result the CT-FFR value is not kept (pressing it clears a typed value): the impression records neither the value nor any discordance. Add them when editing the draft.
With the slider, the report writes only the figure (stenosis, restenosis or graft), without the grade term or range. Add them when editing the draft.
Of two lesions in one segment only the higher grade is described unless the other has features, and a plaque without stenosis loses its type. Describe them when editing the draft.
A lesion drawn across a segment boundary is recorded in both segments with the same grade and features. Check the grade of each segment.
The impression names the distal LCx segment «LCx mid-distal» (SCCT 2026 has no mid LCx). Write «LCx distal» when editing the draft.
With right dominance the map does not show the L-PLB (segment 18): a lesion in a posterolateral branch of the LCx cannot be marked and does not count in the category or the SIS. If there is one, add it when editing the draft and check the category.
A lesion marked with a single tap on the segment that holds the anastomosis of a patent graft has no position: the map cannot tell whether it lies proximal to the anastomosis (bypassed, not coded) or at or beyond it (coded), so it counts it in the category. Drag along the vessel to draw where it lies, and check the category.
They keep the text of the warning as it was before the fix.
With a patent graft, lesions distal to the anastomosis are not counted (e.g. 1/P2/G instead of 4A/P2/G). Assign the category manually.
Marking N deletes the plaque in that segment (including native plaque outside a stent); removing N does not restore it, and SIS/P/report change.
A narrowing without plaque (anomaly, intramural course, bridge, dissection) is not graded: e.g. 0/E instead of 3/E; drawn as a lesion it is written as plaque and adds P1.
The report prints a systolic compression value (45% by default) even if compression was not assessed.
Graft stenosis set with a button prints an internal number, not a measurement or range.
The ramus intermedius is present by default and, with unmarked segments described as normal, the report describes it as normal although nobody marked it. If there is no ramus, switch off «Ramus intermedius present».
Clicking the first ~20% of the LAD with a mouse may open the LCx/RI instead.
SYNTAX and CAD-RADS share the code format: a SYNTAX case opened in CAD-RADS invents high-risk plaque features.
A total occlusion drawn across several segments is weighted ×5 in each segment.
Numbers typed with non-ASCII digits or separators (Arabic, Devanagari, full-width) are truncated or ignored.
Pre-TAVI contrast volume has no weight-based ceiling or warning.
