本用語集は、冠動脈CT(冠動脈CTアンギオグラフィ、CCTA)の読影と、教育用ツールであるcctamapの冠動脈マップの使用に必要な用語をまとめたものである。対象はセグメント解剖、プラーク、狭窄、画質、CAD-RADS 2.0のカテゴリーと修飾子である。各用語には定義と読影ワークステーションでの使い方を記し、多くの用語には陥りやすい誤りと、ワンクリックでマップに表示できる症例を添えた。
定義はSCCT 2026コンセンサスとCAD-RADS 2.0に準拠し、各用語に根拠となる節・表・ページを示した。心臓CT用語全体の基準は、2023年に改訂されたSCCTの標準用語文書である。本用語集は学術資料であり、ガイドラインや臨床判断に代わるものではない。

- Agatstonスコア CAC(CACS)
- 各冠動脈病変について、130 HUの閾値を超える石灰の面積に、その最大CT値に応じた1–4の係数を乗じる。
- CAD-RADSカテゴリー(患者単位) CAD-RADS
- CAD-RADS 2.0では、患者1人につき1つのカテゴリーを付与する。
- CAD-RADSの記載順と修飾子 P・N・HRP・I・S・G・E
- CAD-RADS 2.0のコードは、狭窄カテゴリー、プラーク負荷、修飾子の順に「/」で区切って記載する。
- CAD-RADS(安定胸痛)
- CAD-RADS 2.0は、安定胸痛患者を対象に、各カテゴリーに推奨管理を対応させている。
- CAD-RADS 0とCAD-RADS 1
- CAD-RADS 0とするのは、プラークも狭窄もない場合に限られる。
- CAD-RADS 2とCAD-RADS 3
- CAD-RADS 2は、最大狭窄が軽度狭窄(25–49%)にとどまる非閉塞性病変である。
- CAD-RADS 4Aと4B 4A・4B
- CAD-RADS 4Aは1枝または2枝の高度狭窄(70–99%)である。
- CAD-RADS 5(完全閉塞)
- CAD-RADS 5は、少なくとも1か所の完全閉塞(100%)を示す。

- 異常冠動脈の走行
- 走行は、異常冠動脈が心基部のどこを通過するかを示す。
- 位置ずれ(階段状)アーチファクト
- 異なる心拍で撮影されたスラブの境界で、構造が急にずれる現象である。
- 鋭縁枝 AM(SCCT番号なし)
- 鋭縁枝は、RCA中間部の終点から心臓の鋭縁までの間でRCAから起始する。

- 完全閉塞 100%
- 完全閉塞は、内腔が造影されない冠動脈セグメントであり、100%狭窄に相当する。
- 冠動脈起始異常 AAOCA・ALCAPA・ARCAPA
- 冠動脈が通常の部位以外から起始するものを起始異常という。
- 冠動脈18セグメントモデル(SCCT)
- SCCTは冠動脈CTの読影用にAHAのセグメントモデルを改変した。
- 冠動脈優位性
- 優位性は後下行枝の起始で決まる。
- 冠動脈瘤と冠動脈拡張症 CAA・CAE
- 冠動脈瘤は、径が隣接する正常セグメントの1.5倍を超え、血管長の50%未満にとどまる拡張である。
- 画像ノイズ
- 光子数が少ないと画像は粒状となり、明暗の線状影がランダムに現れる。
- 起始異常の高リスク解剖
- 動脈間走行、壁内走行、スリット状の入口部、鋭角な起始角度、近位部の狭小化、高位起始は、虚血と関連するため高リスクとされる。
- 狭窄度分類(0–5) 0–5
- SCCT 2026とCAD-RADS 2.0は、径狭窄率を狭窄なし(0%)、軽微(1–24%)、軽度(25–49%)、中等度(50–69%)、高度(70–99%)、完全閉塞(100%)の6段階に分類する。
- 狭窄のレポート記載
- 各狭窄は、解剖学的部位、原因となるプラークの種類、用語と範囲で表した狭窄度によって記載する。
- グラフト狭窄とグラフト閉塞
- グラフト狭窄は自己冠動脈の狭窄と同様に評価し、その部位は吻合部またはグラフト体部である。
- 径狭窄率 %DS
- 径狭窄率は、最狭窄部の内腔径が正常な参照セグメントの内腔径に比べて減少した割合を百分率で表したものである。
- 検査画質
- 検査全体の画質には確立された基準がないため、SCCT 2026は視覚的スケールでの評価を推奨している。
- 後下行枝と後側壁枝(右・左) R-PDA・L-PDA・R-PL・L-PL (SCCT 4/15/16/18)
- 後下行枝(RCA由来のR-PDA、LCX由来のL-PDA)は後室間溝を走行する。
- 高リスクプラーク HRP
- 4つの所見(低吸収プラーク、陽性リモデリング、斑状石灰化、ナプキンリング徴候)のうち2つ以上を明らかに示すプラークを、高リスクプラーク(HRP)とする。
さ行
- 参照血管径 RLD
- 参照血管径は、狭窄近傍の近位側または遠位側にある正常セグメントの内腔径、あるいは両者の平均である。
- 視覚的評価と定量的計測
- 視覚的評価では、読影医が内腔を参照部位と目視で比較して狭窄の範囲を判定する。
- 修飾子I(CT-FFR) I+・I±・I−
- CT-FFRを用いる場合、修飾子Iは病変特異的な値で判定する。
- 修飾子I(負荷CTパーフュージョン) CTP
- CTパーフュージョンでは、可逆性の欠損、または安静時より負荷時に大きい欠損をI+とする。
- 修飾子E(例外) E
- 修飾子Eは非動脈硬化性の冠動脈所見を示す。
- 修飾子S(ステント) S
- Sは、冠動脈のいずれかの部位に少なくとも1本のステントが存在することを示す。
- 修飾子G(グラフト) G
- Gは、少なくとも1本の冠動脈バイパスグラフトが存在することを示す。
- 心筋ブリッジ MB
- 心外膜冠動脈の一部の区間が心筋内のトンネルを走行する状態であり、その上を覆う心筋束をブリッジと呼ぶ。
- 心筋ブリッジの深さ・長さ・収縮期圧迫
- SCCT 2026は心筋ブリッジを深さと長さで分類する。
- ステント内完全閉塞
- ステント内腔の一部の区間に造影剤を認めない場合を、ステント内完全閉塞とする。
- ステント内再狭窄 ISR
- ステント内再狭窄は、ステント内またはその辺縁における内腔の狭小化である。
- セグメント関与スコア SIS
- SISは、プラークの組成や狭窄度にかかわらず、プラークのある冠動脈セグメントを数えるスコアである。
- セグメント境界・分岐部の病変の帰属
- 分岐部を含め、2つのセグメントの境界にある病変は、血流方向で手前にある近位側のセグメントに帰属させる。
- 石灰化プラーク
- 石灰化プラークでは、プラーク容積の大部分が造影された内腔よりも高いCT値を示す。

- 対角枝(D1・D2) D1/D2 (SCCT 9/10)
- D1とD2は、左室前側壁を走行する対角枝のうち最大の2本である。
- 大伏在静脈グラフト SVG
- 静脈グラフトは大伏在静脈の一部を用いたフリーグラフトで、上行大動脈と冠動脈を結ぶ。
- 中間枝 RI (SCCT 17)
- 中間枝は、左主幹部が二分岐ではなく三分岐する場合に、LADとLCXの間で左主幹部から直接起始する枝である。
- 中枢側・末梢側吻合部と末梢ランオフ
- 中枢側吻合部はグラフトを流入血管に、末梢側吻合部はグラフトを標的冠動脈に接続する部位である。
- 直列狭窄とびまん性病変
- 同一血管に正常内腔を挟んで複数の狭窄が存在するものを、直列病変または多発病変という。
- 低吸収プラーク LAP
- 低吸収プラークは、内部のCT値が30 HU未満の非石灰化プラークであり、高リスク所見の1つである。
- 動脈グラフト(内胸動脈・橈骨動脈) LITA/LIMA
- 動脈グラフトは患者自身の動脈をグラフト材料として用いる。
- 鈍縁枝(OM1・OM2) OM1/OM2 (SCCT 12/14)
- 鈍縁枝はLCXから起始し、左室側壁を走行する。
な行
- 内腔の造影不良
- 体格、循環時間の延長、注入手技、造影剤の血管外漏出などにより、内腔が十分に造影されない状態である。
- ナプキンリング徴候 NRS
- ナプキンリング徴候は、非石灰化プラークの短軸像でみられる高リスク所見である。
は行
- 斑状石灰化 SC
- 斑状石灰化は、プラーク内にある直径3 mm未満の石灰沈着である。
- バイパスされた自己冠動脈セグメント
- グラフトの末梢側吻合部より近位にある自己冠動脈セグメントを、バイパスされたセグメントとする。
- 非石灰化プラーク NCP
- 非石灰化プラークは、造影された内腔よりも低いCT値を示す。
- 左回旋枝の近位部・遠位部 LCX (SCCT 11/13)
- LCX近位部は左主幹部の終点からOM1の起始部まで、遠位部は房室間溝内を走行する残りの部分である。
- 左主幹部 LMT (SCCT 5)
- 左主幹部(LMT)はSCCTのセグメント5(AHA分類でも#5)である。
- 左前下行枝の近位部・中間部・遠位部 LAD (SCCT 6–8)
- LAD近位部は左主幹部の終点から最初の大きな中隔枝まで、中間部はそこから心尖部までの距離の1/2まで、遠位部はそれより先の残りの部分である。
- 評価可能なステントと評価不能なステント
- ステント内腔が再狭窄を評価できる程度に明瞭に描出されていれば、そのステントは評価可能である。
- 評価対象とする最小血管径 ≥1.5 mm
- ガイドラインは、画質良好な検査では径1.5 mm以上、標準的な画質の検査では径2.0 mm以上のすべての分枝を評価するよう求めている。
- 評価不能(N)のカテゴリーと修飾子 N
- Nは、確信をもって評価できない径1.5 mm以上のセグメントを示す。
- 評価不能区域
- 血管径1.5 mm以上のセグメントのうち、アーチファクトのために確信をもって読影できないものをいう。
- ビームハードニングアーチファクト
- 金属、石灰化、高濃度の造影剤は低エネルギー光子を選択的に吸収する。
- 部分石灰化プラーク
- 石灰化成分と非石灰化成分をあわせもつプラークである。
- ブルーミングアーチファクト
- 各ボクセル内の部分容積効果により、内腔と石灰化またはステントとの境界がぼける。
- プラーク負荷(P1–P4) P1–P4
- プラーク負荷は、狭窄とは独立した冠動脈プラークの量である。
- 閉塞性狭窄と非閉塞性狭窄 ≥50%・≥70%
- 50%以上の狭窄を閉塞性、50%未満の狭窄を非閉塞性と呼ぶ。
- 壁内走行
- 壁内走行とは、異常冠動脈の近位部のうち、大動脈壁から離れる前に大動脈壁内を走行する部分をいう。
ま行
- 右冠動脈の近位部・中間部・遠位部 RCA (SCCT 1–3)
- RCA近位部は入口部から右室鋭縁までの距離の1/2まで、中間部は鋭縁まで、遠位部はR-PDAの起始部までである。
- モーションアーチファクト
- 撮影中の心拍動、呼吸、体動により血管の輪郭がぼけるアーチファクトである。

- 陽性リモデリング PR
- プラーク部の血管外径が参照径より10%を超えて大きい場合(リモデリング指数1.1超)を陽性リモデリングとする。
